With direct access in Indiana, no referral is needed to see a Physical Therapist. Seeing a Physical Therapist first can often save you hundreds or even thousands of dollars. Feel free to call us if you have more questions concerning direct access. 

If a physician was seen prior to your initial visit, then a referral would be helpful along with your payment information.  If utilizing insurance to pay for services, then bring your insurance information with you along with valid driver's license. Auto insurance claims and Work Compensation need a claim number along with case manager's information if applicable. All other forms will be available at the front desk or may be printed off online prior to your first visit. 

We work with the majority of insurance carriers, but please reach out to your insurance provider if we do not carry them at this time. As we continue to grow as a company, we plan to continue adding insurance carriers. Before your first visit you will know what services are covered. And if we do not accept your insurance, we offer reasonable cash rates as an alternative.

- We are not in network with Parkview Signature Care, following multiple attempts. 

- We will also not be accepting Medicaid at this time.

Yes, Initial appointments will cost $150, while follow up visits will cost $100. This will typically be for one full hour with the therapist. 

You may send in a check to the provided address or call in with your card to pay over the phone. Note that the pay over the phone feature will only be available during normal business hours. 

Walk-ins are accepted at this time. If we do not have immediate availability upon arrival, we will try our best to accommodate you with an appointment within 24 hours. 

Due to our partnership with CWC we are able to provide herbal and vitamin supplementation to improve recovery speed and heal from injury without use of OTC medication.
